Adult Behavioral Health Home and Community Based Services (BH HCBS) were created and developed collaboratively by New York State's Office of Mental Health, Office of Alcoholism and Substance Abuse Services and the Department of Health as a part of the State's Health and Recovery Plan (HARP) for individuals with serious mental illness and substance use disorders.
Venture Forthe BH HCBS are for adults 21 years and older.
The intent of PSR is to restore an individual's
independent living, health, community, social, wellness and personal care
skills. This service can be provided in the home or community. A goal may
include the individual becoming an active and productive member their family or
community and/or culture. An example of this is: if the person has struggled
with keeping friends since their recovery process began, the PSR staff member
could help teach social skills and help with pursuing recreational activities.
Eligibility Criteria:
An individual must have the desire and willingness to receive rehabilitation and recovery services to learn skills to support their long-term recovery for substance use or mental illness.
Eligibility Criteria:
The service is intended
to aid individuals with a behavioral health diagnosis who have had limited
success in outpatient treatment. It is also for those who wish to have their
families more involved in their treatment. Individuals who are being discharged
from an inpatient unit, jail or prison, are transitioning
from crisis services, are no longer involved in a treatment program, or who may have a history of non-engagement in
services can benefit from CPST.
Habilitation services are designed to assist individuals
with a behavioral health diagnosis in learning skills such as communication,
self-help, domestic self-care, socialization, fine and gross motor skills, mobility,
personal adjustment, relationship development, use of community resources and
adaptive skills necessary to reside successfully in home and community-based
settings. This service can be provided in the home or community. For example,
if the person does not have transportation, their staff member could teach them
how to use public transportation. Or, if the individual has trouble managing
their money, their staff member could teach them budgeting skills.
Eligibility Criteria:
The individual receiving the service should be in need of learning skills that are necessary to community living.
A service which partners with the individual's family and
other supports to provide instruction on treatment regimens, elements, recovery
support options, recovery concepts, and medication education for their loved
one.

individual has expressed a desire to include their family or friends in their
recovery process and wishes to utilize them as a support system.

must have a clear desire to work in a competitive employment environment.
Eligibility Criteria: The individual must have made a clear decision to work in competitive employment in the community.
Eligibility Criteria: The individual must have made a clear goal to continue to work in a competitive employment environment.
