Personalized Care Plans for Independence


Our clinical services provide support to individuals who want to maximize their independence after a life-altering event, such as stroke, accident, or when suffering from Alzheimer's disease or dementia.

Custom care programs include support for those participating in the New York State Traumatic Brain Injury (TBI) or Nursing Home Transition Diversion (NHTD) waiver programs and a highly-rated dementia care program.

 



Independent Living Skills Training (ILST)

This program teaches clients the skillset needed to live independently in their homes and communities. Services include self-care, medication management, task completion, communication and social skills, sensory and motor skills, transportation, reduction or elimination of maladaptive behaviors, problem solving and money management.


Community Integration Counseling (CIC)

This service is offered to both clients and their families, and helps them cope with challenges that accompany the client's altered abilities. Services can be one-on-one or in a group setting.


Positive Behavioral Intervention and Support (PBIS)

This program is ideal for individuals with behavioral difficulties. Services are provided both in the client's home and community. The goal of this program is to teach appropriate behaviors and reduce intensity and frequency of behavioral difficulties.


Structured Day and Social Day

Held in a supervised environment, participants enjoy volunteer work, community events and recreational outings.

 


Dementia Care

Together with an in-home caregiver and a memory care counselor, this program supports those living with Alzheimer's or dementia. An in-home caregiver will provide care and companionship. A memory care counselor will work with the client and his or her family to improve quality of life and help develop ways to cope with and understand the disease.

All of our memory care counselors are Certified Dementia Practitioners, helping individuals adjust as their cognitive abilities change.

Our program uses a fail-free approach, meaning the participant is never wrong. This ensures a supportive environment for the client's self esteem.
