Participants and their families manage and execute care plans to meet the unique needs of the participant. This includes hiring, scheduling and training caregivers. Let Your Loved Ones Get Paid for Your Care With CDPAP

Your loved ones may already be taking on much of your daily care. With CDPAP, they can provide a higher level of care and get paid to do it. This program is 100% funded by Medicaid, so your loved ones can get paid to do the things they’re already helping you with.

Have Confidence in Your Care

Hire someone you already know and trust to be your caregiver, and be confident that you’re getting the personal, compassionate care you need from someone who understands your needs on an intimate level. Choose an adult child, relative or friend (anyone but a spouse) to get paid to do the things a personal care aide would handle:

Laundry
Light cleaning
Grocery shopping
Cooking

Dressing and grooming
Community engagement
Medicine administration
Other daily tasks and activities

Support When You Need It

Through CDPAP, you are responsible for hiring and supervising your own caregiver, but Venture Forthe is available for administrative and fiscal support. We’re also available to assist with required paperwork, to serve as a fiscal intermediary (required by Medicaid), and to answer any questions you might have.

The CDPAP Process

Eligibility Assessment

Your eligibility assessment includes two parts set up through Maximus. First, you will be assessed by an independent practitioner. Second, a Registered Nurse from Maximus will conduct a community health assessment. After these assessments have been completed, your insurance agency will develop a plan of care and determine the number of hours of service and issue an authorization.

Enrollment

If you qualify for CDPAP services, Venture Forthe is available to assist you with Medicaid enrollment and serve as your fiscal intermediary, assisting with billing and processing payment of your caregiver.

Choose Your Caregiver

Choose any trusted friend or relative (anyone except a spouse) to serve as your caregiver. Caregivers don’t need any special training or certification, just a desire to provide the compassionate care you need.

Enjoy Life At Home

With CDPAP, you serve as the manager of your own care. Your chosen caregiver will get paid, through Medicaid, to provide the home care you need, the way you need it. You get care from someone you trust, they get paid for their time.
