WHAT IS CDPAP?
CDPAP (Consumer-Directed Personal Assistance Program) is a Medicaid program that lets those who need care choose their own caregiver—including a loved one.
Just as an elder or parent may have long cared for their child or grandchildren, this program makes role reversal doable, ensuring that loved ones can get paid for the care they provide.

How our process works
We help simplify the CDPAP process and help you work through these steps:

ELIGIBILITY
ASSESSMENT
The eligibility assessment includes two parts: assessment by an independent practitioner and a community health assessment. These are done by New York State. Once completed, your insurance agency will develop a plan of care and determine the number of hours of service and issue an authorization.

ENROLLMENT
ASSISTANCE
If you qualify for CDPAP, we can help with enrollment and serve as your fiscal intermediary, assisting with billing and caregiver payment.

CAREGIVER
CHOICE
Any trusted friend or relative (with the exception of a spouse) can be a caregiver—there’s no special training or certification needed, just a commitment to compassionate care.
