Venture Forthe is an official Medicaid home health care service provider. We proudly provide care under many Medicaid-funded programs, based on eligibility and individual needs. TBI Waiver Program
The TBI (traumatic brain injury) Waiver is a Medicaid-funded program designed to allow individuals who have suffered a traumatic brain injury or stroke to stay in or return to their homes and continue to make choices about their care. Waiver programs are specifically for people who would otherwise be in an institution (nursing home, hospital) for long-term care.
NHTD Waiver Program
The Nursing Home Transition & Diversion Waiver program (NHTD) uses Medicaid funding to provide support and services to individuals who are nursing home eligible, but would prefer to stay in or return to their homes. Waiver programs are specifically for people who would otherwise be in an institution (nursing home, hospital) for long-term care.
HARP (Health & Recovery Plan) is a health care program under Medicaid Managed Care that provides overall health care for adults with significant behavioral and/or mental health needs.
HCBS (Home & Community Based Services) provide in-home support services for adults with intellectual or developmental disabilities, physical disabilities, or mental illness. HCBS are offered through HARP on a tiered basis, depending on individual needs.
Health Home Services
A Health Home is a care management service that unites each party involved in an individual’s care. Health Home Management is ideal for Medicaid beneficiaries who have multiple or chronic health problems that make it difficult to coordinate and receive the care needed to get and stay healthy.
County Medicaid Services
Enrolling in Medicaid through your local Department of Social Services allows for combination of services that may not be possible through state Medicaid. If you are currently enrolled in Medicaid through your county, it may be in your best interest to continue receiving care under those services.
