Those living with a mental health disorder need community based services that focus on providing the full spectrum of assistance required to be contributing members of their communities.

Our behavioral health services are ideal for individuals who have struggled with other treatment and rehabilitation options, and require specialized, compassionate, in-home care and support.

Complete a Referral for CORE Services

Once submitted, one of our Outreach Coordinators will respond within a 24 hour period to confirm client eligibility and make contact with the client within 72 hours to begin our intake process. Who is Eligible for CORE?

HCBS eligibility is determined through a state eligibility assessment. Some qualifying criteria include:

  • Have a Managed Medicaid Plan
  • 21 years of age or older
  • Have a qualifying behavioral health diagnosis (serious mental health and/or substance use diagnosis)
  • Prequalifying coding designated by NYS Medicaid
