Professional Care and Guidance for Memory Loss

Our specialty in memory care ensures that your loved one is taken care of in a compassionate and professional manner. Each participant in our Alzheimer’s and Dementia Care program will be assessed by one of our Certified Dementia Practitioners, and based off of this assessment, feedback will be given to our nurses in drafting a care plan that is individualized to a participant’s needs. Our CDPs understand how to redirect challenging behaviors, such as mood swings, anxiety or depression, and incorporate meaningful activities into a person’s daily routine. Venture Forthe understands the specific needs of individuals who are experiencing memory loss and we are here to answer any questions you or your loved one may have.

Our Certified Dementia Practitioners will work with you and your family members to develop a comprehensive plan focusing on the needs of your loved one and are there to provide insight to our staff and families whenever it is needed. Venture Forthe understands the significance of providing at-home care to your loved ones, and we are committed to earning and keeping your trust. We strive to do everything we can to assist you and your loved one through the process of choosing and enrolling in home health care services.

Available On-Call 24/7

Venture Forthe has staff, including a registered nurse, on-call 24 hours a day, and are able to provide round-the-clock, in-home care for those who need it. We are here to assist you, no matter what the issue.

Enrollment Support

Venture Forthe is able to offer assistance in understanding the home care service options available. We help our clients gain access to the benefits and services they’re eligible for, while streamlining the enrollment process.

Service Coordination

Venture Forthe has dedicated service coordinators to assist with obtaining services that meet each need. The service coordinator monitors all services to ensure that each individual receives the high-quality, comprehensive care they need.

Highly Trained Staff

Our staff members are fully trained in safety and compliance on a regular basis. Each staff member has the skills necessary to ensure that each client is safe and secure in the care that they are receiving.

Certified Memory Care Practitioners

Our Certified Dementia Practitioners are individuals who are there to provide appropriate support, competency, sensitivity, and act as a resource to family members and the participant as cognitive functioning declines. The CDP certification represents that our staff and health care professional has received comprehensive knowledge in the area of dementia care, achievement in completing the Alzheimer’s Disease and Dementia Care course/seminar, and met the NCCDP requirements for CDP certification.

Personal Care Aides

PCAs (Personal Care Aides) are individuals who come to your home and perform the in-home aspects of a personal care plan. Each Venture Forthe PCA is certified, trained and thoroughly screened. PCAs are overseen by registered nurses, and can assist with:

  • Laundry & cleaning
  • Cooking & grocery shopping
  • Getting around
  • Dressing, bathing & grooming
  • Medication prompts
  • Companionship

Registered Nurses

Each Venture Forthe client receives a consultation with a registered nurse to evaluate medical needs. RNs and PCAs work together to ensure proper care that meets those needs is delivered. There will always be an RN on call 24 hours a day available to consult with you, your loved ones, and their PCA(s).

Eligibility Guidelines

Venture Forthe provides care based on Medicaid eligibility requirements. We also offer care on a private-pay basis for those who are not enrolled in Medicaid or have private insurance. If you believe you’re eligible for home health care, please contact us.
