Venture Forthe offers care through a number of NYS Medicaid-funded systems. For information on enrolling in Medicaid, please call us. We can direct you towards a financial advisor or other professional assistance.
MMC (Medicaid Managed Care), often obtained through the New York State Marketplace, is a health care delivery system that provides health benefits and additional health care services. MMC plans cover a comprehensive range of personal needs. It may cover not only Personal Care Services, but also Behavioral Health (CORE/HCBS) Services at Venture Forthe.
MLTC (Managed Long Term Care) is a Medicaid program designed to deliver long-term services to individuals who are elderly, chronically ill, or disabled, but want to remain in their homes. These individuals are often referred to as dual eligible because they qualify for both Medicare and Medicaid. At Venture Forthe, MLTC may cover long-term home health care for seniors or those with a chronic illness or disability.
Medicaid FFS (Fee-For-Service) provides home health care through certain waiver programs or through county social services. At Venture Forthe, FFS may cover long-term care for those eligible for the Traumatic Brain Injury or Nursing Home Transition and Diversion Waiver programs, as well as certain combination plans.
In addition to Medicaid payers, Venture Forthe is able to provide services through private insurance plans and private pay plans. If you’re unsure of your financing and/or insurance options, please contact us and we’ll be happy to offer guidance.
For those eligible for or currently receiving workers’ compensation insurance benefits, Venture Forthe works with your insurance payer to provide the home care services for which you are eligible.
If you’re enrolled in a Long Term Care plan through your private insurance company, Venture Forthe is able to provide the care you need. We’ll work with your insurance company to develop a care plan.
For those who wish to pay for home health care services out-of-pocket, we accept direct payments as well.
