The TBI (traumatic brain injury) Waiver is a Medicaid-funded program designed to allow individuals who have suffered a traumatic brain injury or stroke to stay in or return to their homes and continue to make choices about their care. Waiver programs are specifically for people who would otherwise be in an institution (nursing home, hospital) for long-term care.
The Nursing Home Transition & Diversion Waiver program (NHTD) uses Medicaid funding to provide support and services to individuals who are nursing home eligible, but would prefer to stay in or return to their homes. CORE (Community Oriented Recovery and Empowerment) Services are person-centered, recovery-oriented, mobile behavioral health supports intended to build skills and self-efficacy that promote and facilitate community participation and independence.
HCBS (Home & Community Based Services) provide in-home support services for adults with mental illness. HCBS are offered through CORE on a tiered basis, depending on individual needs.
Enrolling in Medicaid through your local Department of Social Services allows for combination of services that may not be possible through state Medicaid. If you are currently enrolled in Medicaid through your county, it may be in your best interest to continue receiving care under those services.
