Venture Forthe provides services funded through Health First Colorado, Colorado’s Medicaid program. Available programs include several Home and Community-Based Services (HCBS). Waiver and the Community First Choice (CFC) Option. Each program has specific eligibility and service requirements. Venture Forthe offers care through a number of CO Medicaid-funded systems. For information on enrolling in Medicaid, please call us. We can direct you towards a financial advisor or other professional assistance.
Colorado’s Elderly, Blind, and Disabled (EBD) Waiver helps eligible Health First Colorado members receive long-term services and supports in their homes and communities instead of a nursing facility. Through the EBD Waiver, Venture Forthe specializes in providing Respite Care, which provides temporary support when a primary caregiver needs relief, and Life Skills Training, which helps members develop or maintain skills needed to live more independently – such as self-care, household management, communication, safety awareness, and accessing community resources.
Colorado’s Community Mental Health Services (CMHS) Waiver helps eligible Health First Colorado members living with mental health conditions receive services that support independence and continued community living. Through the CMHS Waiver, Venture Forthe specializes in providing Life Skills Training, providing individualized support to help members build or maintain skills such as self -care, household and time management, communication, safety awareness, problem – solving, and accessing community.
Colorado’s Complementary and Integrative Health (CIH) Waiver helps eligible Health First Colorado members with qualifying physical disabilities receive long-term services and supports in their homes and communities. Through the CIH Waiver, Venture Forthe specializes in providing Respite Care, providing temporary support when a primary caregiver needs relief, and Life Skills Training, helping members build or maintain skills such as self-care, household management, communication, safety awareness, and accessing community resources.
Colorado’s Supported Living Services (SLS) Waiver helps eligible adults with intellectual or developmental disabilities receive services and supports in their homes and communities. Through the SLS Waiver, Venture Forthe specializes in providing Life Skills Training, providing individualized support to help members build or maintain skills such as self-care, household and time management, communication, safety awareness, problem-solving, and accessing community resources.
Colorado’s Brain Injury (BI) Waiver helps eligible Health First Colorado members with a traumatic or acquired brain injury receive services and supports in their homes and communities instead of hospital or nursing facility. Through the BI Waiver, Venture Forthe specializes in providing Respite Care, providing temporary support when a primary caregiver needs relief while helping the member remain safely care for in a familiar setting.
Colorado’s Community First Choice (CFC) option is a Health First Colorado Medicaid State Plan benefit that helps eligible members receive attendant services in their homes and communities. Venture Forthe offers agency – based Personal Care and Homemaker services, as well as In – Home Support Services (IHSS) – a participant – directed service delivery option through which members can direct their Personal Care, Homemaker, and Health Maintenance Activities with support from a licensed IHSS agency. These activities help members manage daily activities, household tasks, and health – related needs while maintaining independence at home.
In addition to Medicaid payers, Venture Forthe is able to provide services through private insurance plans and private pay plans. If you’re unsure of your financing and/or insurance options, please contact us and we’ll be happy to offer guidance.
For those eligible for or currently receiving workers’ compensation insurance benefits, Venture Forthe works with your insurance payer to provide the home care services for which you are eligible.
If you’re enrolled in a Long Term Care plan through your private insurance company, Venture Forthe is able to provide the care you need. We’ll work with your insurance company to develop a care plan.
For those who wish to pay for home health care services out-of-pocket, we accept direct payments as well.
