Community First Choice (CFC) is a Health First Colorado (Colorado Medicaid) State Plan benefit that helps eligible members receive attendant services and supports in their homes and communities. CFC promotes choice, independence, and person-centered care while allowing members to receive authorized services through different service delivery options.
Venture Forthe provides both agency-based services and In-Home Support Services (IHSS), giving eligible members options for how their care is provided.

Assistance with Everyday Activities
Personal Care provides hands-on assistance with activities of daily living based on your assessed needs and authorized service plan. Caregivers provide respectful, dependable support while encouraging independence whenever possible.

Homemaker Services assist with routine household tasks that support a safe, healthy, and comfortable home environment. Services are based on your assessed needs and authorized service plan.

Direct Your Care with the Support of a Licensed IHSS Agency
In-Home Support Services (IHSS) is a participant-directed service delivery option available through Community First Choice. IHSS gives you or your Authorized Representative greater choice and control over your attendants and how authorized services are provided, while Venture Forthe provides the administrative support and required agency oversight.
Through IHSS, authorized services may include Personal Care, Homemaker Services, and Health Maintenance Activities.

Community First Choice is available to eligible Health First Colorado members who meet applicable program requirements and have an assessed need for covered services.
Eligibility and authorized services are determined through Health First Colorado and the member’s Case Management Agency based on individual needs and program requirements.
