Venture Forthe provides services funded through Health First Colorado, Colorado’s Medicaid program. Available programs include Community First Choice (CFC) and several Home and Community-Based Services (HCBS) Waviers. Each program has specific eligibility and service requirements. Community First Choice (CFC) is a Health First Colorado Medicaid State Plan benefit that provides in-home attendant services to help eligible members live safely and independently in their homes and communities. Services may include Personal Care, Homemaker, and Health Maintenance Activities based on each member’s assessed and authorized service plan.
Health First Colorado offers several Home and Community – Based Services (HCBS) Waivers that provide long-term services and supports for eligible members with specific needs. Each waiver serves a particular population and may provide access to authorized in-home and community-based services that help members maintain independence and continue living safely in their homes and communities.
In addition to services funded through Health First Colorado, Venture Forthe accepts a variety of other payment and funding options. If you are eligible for or currently receiving workers’ compensation benefits, Venture Forthe can coordinate with the applicable insurance carrier or claims administrator to confirm authorization and arrange approved home care services.
If you have a long-term care insurance policy, Venture Forthe can help you understand the documentation needed for your claim and coordinate with your insurance carrier when appropriate. Coverage, benefit amounts, and reimbursement requirements depend on the terms of your individual policy.
Venture Forthe offers private pay services for individuals who choose to pay for home care directly. We will work with you to identify services and scheduling that best meet your needs and provide information about applicable rates and payment terms.
