The Community Mental Health Supports (CMHS) Waiver is a Health First Colorado Home and Community-Based Service (HCBS) Wavier that helps eligible members receive long-term services and supports in their homes and communities.
Venture Forthe provides Life Skills Training for eligible CMHS Waiver members, with personalized support focused on building everyday skills, increasing independence, and working toward meaningful indivdidual goals.
Life Skills Training provides individualized support to help members develop and strengthen skills for greater independence at home and in the community. Services are personalized around each member’s strengths, needs, and goals.
Support may focus on daily routines, household organization, meal planning, budgeting, community living, planning and organization, communication, self-advocacy, problem-solving, and other skills based on individual goals and authorized needs.
The CMHS Waiver is available to eligible Health First Colorado members who meet applicable program requirements and have qualifying needs for long-term services and supports.
Eligibility is determined through Health First Colorado and the appropriate assessment and case management process based on individual circumstances and program requirements.
