The Elderly, Blind, and Disabled (EBD) Waiver is a Health First Colorado Home and Community-Based Services (HCBS) Waiver that helps eligible members receive long-term services and supports in their homes and communities.
Venture Forthe provides Life Skills Training and Respite Care for eligible EBD Waiver members, with services focused on supporting independence, everyday living, and quality of life at home.
Life Skills Training provides personalized support to help members build and strengthen skills for greater independence at home and in the community. Support may focus on daily routines, household organization, meal planning, budgeting, community living, communication, problem-solving, and other skills based on individual goals and authorized needs.
Respite Care provides temporary, dependable support when a primary caregiver needs time to rest, recharge, or manage other responsibilities. Services may include personal care, companionship, meal preparation, mobility support, safety supervision, and other authorized assistance based on individual needs.
The EBD Waiver is available to eligible Health First Colorado members who meet applicable program requirements and need a qualifying level of long-term services and supports.
Eligibility is determined through Health First Colorado and the appropriate assessment and case management process based on individual circumstances and program requirements.
