The Supported Living Services (SLS) Waiver is a Health First Colorado Home and Community-Based Services (HCBS) Waiver that helps eligible adults with intellectual or developmental disabilities recieve services and supports that promote independence and community living.
Venture Forthe provides Life Skills Training for eligible SLS Waiver members, with personalized support focused on strengthening everyday skills, building confidence, and working toward greater independence.
Life Skills Training provides individualized support to help members develop and strengthen skills for greater independence at home and in the community. Services are personalized around each member’s strengths, needs, and individual goals.
Support may focus on daily routines, household organization, meal planning, budgeting, community living, planning and organization, communications, self-advocacy, problem-solving, and other skills based on individual goals and authorized needs.
The SLS Waiver is available to eligible Health First Colorado members who meet applicable program requirements and have qualifying needs for services and supports.
Eligibility is determined through Health First Colorado and the appropriate assessment and case management process based on individual circumstances and program requirements.
