Managed Long-Term Care (MLTC) is a Medicaid program in New York State that assists individuals who are chronically ill or disabled and wish to remain in their homes and communities rather than move to a nursing facility. Through MLTC, eligible individuals receive a comprehensive range of Long-Term Services and Supports (LTSS), such as home care, personal assistance, therapies, and adult day care, coordinated by managed care plans approved by the New York State Department of Health. According to NY State, “the entire array of services to which an enrolled member is entitled can be received through the MLTC plan the member has chosen.”
