What is Managed Long-Term Care in New York State?

Managed Long-Term Care (MLTC) is a Medicaid program in New York State that assists individuals who are chronically ill or disabled and wish to remain in their homes and communities rather than move to a nursing facility. Through MLTC, eligible individuals receive a comprehensive range of Long-Term Services and Supports (LTSS), such as home care, personal assistance, therapies, and adult day care, coordinated by managed care plans approved by the New York State Department of Health. According to NY State, “the entire array of services to which an enrolled member is entitled can be received through the MLTC plan the member has chosen.”

Eligibility Criteria

To qualify for MLTC, individuals must:

• Be 18 years or older
• Have a chronic illness or disability that qualifies them for nursing home care
• Require community-based long-term care services for more than 120 days
• Have active enrollment in Medicaid
• Be able to remain at home at the time of enrollment safely
• Have approval by New York State through the necessary assessments
