HCBS are specialized services designed to increase independence and manage mental health symptoms. All services are offered from the comfort of your home, allowing you to receive care in the setting that best meets your needs.
Venture Forthe’s Home & Community Based Services (HCBS) provide behavioral health support for adults with mental illness. The services we provide aim to help support you in achieving your mental health goals, whether they be educational, employment, or lifestyle-based. Services are rendered by trained Venture Forthe behavioral health professionals, and offered in your home or another familiar setting of your choosing.
Venture Forthe professionals understand that each of our clients has unique goals and needs, and independence looks different for everyone. If your loved one struggles with day-to-day living, they deserve the opportunity to live a fulfilling, productive life. Our behavioral health and community-based services are designed to offer support for independent living and meaningful community integration. Venture Forthe provides comprehensive life skills training, from qualified professionals, that empowers our clients to live the most independent life possible.
Community Psychiatric Support & Treatment (CPST) is a recovery-focused intervention service that is ideal for individuals who have struggled with site-based care or treatment, and would benefit from in-home counseling. With CPST, participants have the option to receive services alongside family or loved ones, if this would be beneficial to recovery. CPST services are provided by licensed behavioral health professionals, and aim to minimize the negative symptoms of an individual’s diagnosis and facilitate community integration and recovery.
Obtaining and maintaining employment with a behavioral health diagnosis is a way to take steps towards independence and financial opportunity. Meaningful employment also helps individuals develop pride in accomplishments, and become integrated members of their communities. Venture Forthe offers support for each step of this process, from education support to ongoing employment support. This develops the foundational skills and credentials necessary for people of all abilities to reach their vocational goals.
HARP (Health & Recovery Plan) is a health care program under Medicaid Managed Care that provides overall health care for adults with significant behavioral and/or mental health needs. HARP offers the same benefits as regular Medicaid health plans, plus additional benefits and specialized support services. To enroll in HARP, you must be deemed eligible by NYS. If you think you may be eligible or would like to refer someone, please contact Venture Forthe.
HCBS (Home & Community Based Services) provide in-home support services for adults with mental illness. HCBS are offered through HARP on a tiered basis, depending on individual needs. HCBS can help with daily living and social skills, education and employment goals, peer and family support, and symptom management.
HCBS eligibility is determined through a state eligibility assessment. Some qualifying criteria include:
