Through Nevada Medicaid, Venture Forthe helps the community navigate the self-directed home care model.  The self-directed model gives you full control over your in-home care. Through self-directed care, you select and manage your caregivers, set schedules, and direct how your care is delivered, while Venture Forthe handles payroll, compliance, and administrative support. Self Directed Care is part of Nevada Medicaid’s long-term services and supports and allows members to receive personal care and certain skilled services at home. You stay independent with care from people you choose, while Venture Forthe ensures everything runs smoothly behind the scenes.

Self-Directed Care Made Simple

Self-Directed Care is a Medicaid service delivery model that allows you to direct your personal care services at home. With Self-Directed Care, you:

  • Select, hire, and manage your caregivers
  • Set schedules and daily responsibilities
  • Supervise care on a day-to-day basis
  • Receive help with daily living and approved health-related tasks

Venture Forthe supports you by handling administrative responsibilities, but you are in control of your care and your caregivers.

Checking Your Eligibility

To participate in Self-Directed Care, you must:

  • Be enrolled in Nevada Medicaid
  • Qualify for Personal Care Services (PCS) or approved skilled services
  • Have an assessed need for in-home support
  • Receive authorization for services through Medicaid

Services require prior authorization and must be determined medically necessary through assessment.

Care at Home: What’s Covered

These services support both Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs) and must be approved in advance.

All services must be part of an approved Medicaid care plan.

Personal Care Services (PCS)

  • Bathing, grooming, and dressing
  • Toileting and transfers
  • Mobility and eating support

Homemaker & Daily Living Support (IADLs)

  • Light housekeeping
  • Laundry
  • Meal preparation
  • Essential shopping and errands

Self-Directed Skilled Services

  • Certain health or medical tasks authorized by a provider
  • Delivered by a trained personal care assistant under specific conditions

Your Care Team, Your Control

Under this model, you hire, train, schedule, and supervise your caregivers, while Venture Forthe manages administrative responsibilities.

With Self-Directed Care, you are the managing employer of your caregivers. Caregivers must:

  • Be enrolled through a Medicaid-approved provider, such as Venture Forthe
  • Meet program requirements and pass background checks
  • Follow the schedules and duties you assign

Venture Forthe, Inc. serves as the Intermediary Service Organization (ISO), providing administrative and payroll support.

We Handle the Paperwork, You Focus on Care

Venture Forthe supports the administrative side of your care so you can focus on living well. 

We do:

  • Enroll and onboard caregivers
  • Process payroll and tax filings
  • Ensure Medicaid compliance
  • Provide ongoing support for members and caregivers

We do not:

  • Provide home care services directly
  • Choose caregivers for you
  • Determine Medicaid eligibility

Yes. With the self directing care model, you are responsible for selecting, hiring, and managing your caregivers. This can include people you trust, such as friends or family members, as long as they meet Medicaid requirements and complete enrollment.

Venture Forthe serves as the Intermediary Service Organization (ISO) and handles administrative responsibilities like caregiver enrollment, payroll, tax reporting, timesheets, and Medicaid compliance.

Yes. If you are eligible for Nevada Medicaid and approved for services, self-directed care is fully covered. Medicaid pays for services and hours authorized in your care plan.

In this model, you can receive Personal Care Services (PCS) and certain self-directed skilled services, depending on your needs and what Medicaid approves in your care plan.

Your care hours are determined by Medicaid after an assessment. Only the hours approved in your care plan are covered and eligible for payment.
